I noticed your TV (HDMI I assume) is all the way to the left. I wouldn't think it mattered but the only thing I can think of that was different (in how I set it up) was ports 1-4 on the card were in a group and port 5 was extended (it was all the way to the right).

Another thing I wonder if it has to do with the fact its HDMI? Again I see no real reason that this should be the case but maybe using a DVI DP adapter + DVI -> HDMI converter may have better luck?

I know their drivers are iffy as I was having a very similar problem before. In windows i couldn't hit apply if i enabled the other monitor as extended.. doing extended/enabling the 5th monitor in the catalst center also did not work...

Another thing I would try is getting it to where all the displays are active (no display group) and probably in extended desktop mode and then try to create the group from there (it give you a new type of option where you add each display to the display group one by one instead of chosing 3x1, 2x1, 2x2, etc..).

Those are the only other things I could suggest.

Edit: One more shot in the dark here but have you tried doing this in a single card configuration (NO CF)? It also makes me wonder if CF could be limiting it to three displays. Not sure if other people have done dual eyefinity 6 cards + CF and 3+ monitors before. I of course was running off a single eyefinity 6 card.

conter85: In a few days I will be doing a new setup for my dad with another eyefinity 6 card. When I set it up for my sister I was doing it all remotely via teamviewer so I did not take screenshots or anything.

When I do my dads setup:

Single Viewsonic VP2290b - takes 4 inputs for full refresh rate (2x2 display group of 3840x2400)
2 passive + 2 active single link adapters
Dell 3007WFP 2560x1600 (extended desktop)
1 active dual link (USB powered) adapter
Dell 2407WFP 1920x1200 (extended desktop)
1 Active single-link adapter

All 6 ports will be used and I will be using this setup:

Of course the positions will be a bit differen't and all the 1 displays will really be just a single monitor driven by 4 ports.

I will be setting this up on my dad's machine this coming Tuesday. This time when I set it up I can take screenshots of each step to make sure I know exactly what needs to be done. When I did it for my sister remotely I fealt like I was 'fighting' with the drivers and finally was able to get it working.

Wasn't able to take as many screenshots as I wanted as when setting up the display group the only monitor that is active (vp2290b) likes to go blinky and I had to use teamviewer again. The monitor goes to amber and blinks if it gets a mode it doesn't which is in between going from 1 links to 4 links.

Anyway it basically started out with everything in extended desktop:

From there I created a new display group and chose 2x2 and it asked which monitors I wanted to add (after first saying it will disable my extended desktops).

I then created the display group and was simply able to right click on the displays under 'Please select a display' and chose extended and with the 11.2 drivers this gave me absolutely zero problems:

I would suggest giving it a try without crossfire. Are you able to atleast get all the displays in extended desktop with no display groups?
