I'm using 3 Asus VH242H, an XFX 6950(reference model) with drivers 11.5a and a Sapphire MDP to S-Link DVI adapter.
2 monitors are connected with DVI and 1 with the adapter.

The adapter doesn't recognize my monitor and defaults it to "Generic Non-PNP Monitor".
To fix this I have plugged the monitor in DVI and retrieved it's information with Moninfo by extracting it's driver(.INF).
I then reconnected the monitor with the adapter and replaced the default driver with the one created using MonInfo.
I can now select the proper resolutions and refresh rates but when I create an Eyefinity group, the maximum grouped resolution is 4800x900 instead of 5760x1080.
In CCC the monitor concerned is still listed as "Unknown" which I presume would be the cause.

I've searched a lot on different forums and most issues people seem to have relate to bezel compensation and flickering caused by adapters.
